Immunize Connect Ontario (ICON)

Immunize Connect Ontario (ICON) is the Ontario Ministry of Health’s secure online database used by all provincial public health units for managing immunization records.

Individuals can access immunization information and report vaccines to their local public health unit by logging into their ICON profile. It is required by law to report your child’s immunization records to the health unit, according to the Immunization of School Pupils Act, 1990 (ISPA) and the Child Care and Early Years Act, 2014 (CCEYA).


Why should I use ICON?

  • It is a secure way to organize and keep track of your family's immunization records.
  • Convenient 24/7 access to all immunization information on file with the Windsor-Essex County Health Unit (WECHU).
  • You are able to see upcoming or overdue vaccines needed based on the Publicly Funded Immunization Schedules for Ontario.
  • Securely report vaccines to the WECHU that were given by your health care provider (please attach a photo of your record as proof).
  • Download a PDF summary of all immunization information on file with the WECHU which you can print or share digitally.

How do I use ICON?

  • You will need your Ontario Health Card Number or Ontario Immunization ID. If you do not have a health card, please call the WECHU Immunization Department at 519-258-2146 ext. 1222.
  • Every time you or your child gets a vaccine, report it through ICON so that you have a complete record of your immunizations and they are submitted to the WECHU. Remember to attach a photo of the immunization record with your submission.
