How the IPAC lapse was identified?

Referal from Infectious Disease Department


Summary description of the IPAC lapse.

Failure to reprocess (clean, disinfect, sterilize and store) equipment and devices used in patient care services. Failure to follow the standards set by the Provincial Infectious Diseases Advisory Committee (PIDAC) as outlined in "Infection Prevention and Control for Clinical Office Practice" and the Canadian Standards for "Medical Device Reprocessing".

Dec. 20, 2017 Verbal order issued at time of investigation for immediate correction to current practices.

A physician contacted the WECHU on Dec. 9, 2017 concerned about the use of contaminated instrument used on a patient 9 days prior to contacting our office. The physician performed an incision and drainage to the subject and later discovered that the tray being used was washed and disinfected but not autoclaved. The physician discovered that the same tray had been used the day before by another physician to perform sutures on a patient.

IPAC procedures for effective and required cleaning and disinfection of reusable equipment are in compliance with provincial standards to prevent potential transmission of an infectious disease to patients and/or staff members.
