School and Childcare Centre Absenteeism Reporting Form

Administrators must report absenteeism when:

  • The school or centre has ≥20% absenteeism due to illness among students and/or staff, or 
  • There is concern regarding high absenteeism within a specific classroom or group (this can include instances of less than 20%).

How to report absenteeism:

  • Complete the initial absenteeism report at: www.wechu.org/absenteeism-reporting.
  • Once submitted, the Infectious Disease Prevention Department at the WECHU will the administrator within 1 (one) business day to provide further guidance.
  • If you have any questions while submitting this form, or about absenteeism reporting to the WECHU, please call the Infectious Disease Prevention Department at (519) 258-2146 ext. 1420.
